Responsibilities

• Assess patients' needs and develop tailored treatment plans based on their specific conditions.
• Provide therapeutic interventions to assist patients in achieving their rehabilitation goals.
• Conduct discharge planning to ensure a smooth transition for patients returning home or moving to the next level of care.
• Collaborate with multidisciplinary teams
• Educate patients and their families on techniques to improve daily living activities.
• Timely completion of daily and weekly documentation of patient treatments and progress

Requirements

· Active California license (SLP)

· Excellent communication skills, both verbal and written

· A dedication to ongoing professional development and staying current with best practices

· A devoted love for working with geriatric and post-acute populations
