We are seeking a dedicated and knowledgeable Registered Nurse to join our team as a Peritoneal Dialysis/Home Hemodialysis Training Nurse. This position is centered on empowering patients and their families to manage dialysis treatments independently at home. The ideal candidate will possess exceptional teaching skills, a strong commitment to patient care, and a proactive approach to preventing complications.
Key Responsibilities
Patient & Care Partner Training
- Deliver thorough, hands-on training for peritoneal dialysis (PD) and home hemodialysis
- Educate on aseptic techniques, catheter care, fluid exchanges, machine operation, and infection control.
- Customize teaching methods to accommodate various learning preferences, including those of family members and caregivers.
Home Assessments & Follow-Up
- Conduct initial and yearly home visits to assess the suitability of the environment, and ensure proper supply management and technique.
- Provide monthly clinic reviews, telephonic support, and on-call troubleshooting services to address potential issues such as peritonitis.
Case Management
- Monitor patient progress through lab results, vital signs, and treatment adherence.
- Collaborate with a multidisciplinary team, including nephrologists, dietitians, and social workers, to develop personalized care plans.
Quality & Compliance
- Ensure adherence to regulatory standards for home dialysis as per federal and state requirements.
- Maintain detailed documentation in electronic medical records and report any incidents promptly.
Qualifications
- Active RN license.
- RN with experience in PD training.
- Minimum of one-year nursing experience, with expertise in hemodialysis and peritoneal dialysis.
- At least six months of general dialysis experience.
- CDN (Certified Dialysis Nurse) preferred.
- BLS/ACLS required.
