Position Overview
A leading, high-volume healthcare facility in the Houston area is seeking an experienced Manager of Trauma Case Management. Collaborating closely with the departmental Director, you will execute and manage the day-to-day operations of the case management program at the local level. This crucial leadership role encompasses Care Facilitation, Utilization Management, and complex Discharge Planning for the trauma care continuum.
Key Responsibilities

Operational Oversight: Coordinate daily departmental workflows and resource utilization to ensure maximum operational efficiency.

Program Management: Assist leadership in executing the local case management program, focusing on high-quality clinical outcomes and strategic cost management.

Quality Improvement: Participate in collecting, analyzing, and reporting financial and quality data to drive performance improvement strategies.

Care Coordination: Manage discharge planning processes and facilitate interdisciplinary collaboration across clinical teams.
Candidate Qualifications
Strict Requirements (Must-Haves):

Certification: Active Case Manager Certification (CCM or equivalent) is required.

Licensure: Current, valid license to practice as a Registered Nurse (RN) in the state of Texas OR Licensed Master Social Worker (LMSW). LCSW preferred.

Clinical Experience: Minimum five (5) years of experience in case management, utilization management, discharge planning, or a related cost/quality management program.

Hospital Experience: Minimum three (3) years of dedicated experience in hospital-based nursing or acute care social work.

Leadership: Minimum three (3) years of proven supervisory or leadership experience in a healthcare environment.
