Position Overview
A premier, award-winning healthcare network in Houston is seeking an experienced Manager of Case Management to oversee daily departmental operations for Emergency Department (ED) and Observation (OBS) units. Working closely with the Director, you will lead a high-performing team, drive utilization management strategies, and ensure optimal discharge planning and clinical quality outcomes.

Key Responsibilities

Operational Leadership: Manage day-to-day case management operations at the unit level, supervising professional and support staff.

Care Coordination: Act as the primary liaison between physicians, hospitalists, community resources, and nursing staff to streamline patient transitions.

Utilization & Quality Management: Monitor resource allocation, analyze financial and quality data, and implement performance improvement strategies to optimize patient outcomes.

Team Development: Participate in personnel functions (hiring, training, and performance evaluations) and mentor staff to foster a high-performance culture.
Candidate Qualifications
Strict Requirements (Must-Haves):

Licensure: Active Registered Nurse (RN) license in the State of Texas OR Licensed Master Social Worker (LMSW). LCSW preferred.

Certification: Active Case Manager Certification (CCM or equivalent).

Clinical Experience: Minimum five (5) years of experience in utilization management, discharge planning, case management, or cost/quality management.

Hospital Experience: Minimum three (3) years of experience specifically within a hospital-based nursing or social work environment.

Leadership: Minimum three (3) years of demonstrated supervisory or leadership experience in a healthcare setting.

Education: Bachelor’s Degree in Nursing (BSN) or Social Work (BSW) required.

Strong working knowledge of managed care, inpatient/outpatient continuums, and payor requirements.

Proven ability to collaborate effectively with physicians and interdisciplinary medical teams.
