Position Description
Under the supervision of the Medical Director, the Family Medicine Physician will provide comprehensive, community-based primary care to patients across the lifespan—from pediatrics to geriatrics—in a patient-centered setting. This role is ideal for a physician who is passionate about delivering equitable, relationship-driven care and serving as a consistent healthcare partner for individuals and families.
The Family Medicine Physician will care for a diverse, underserved population and play a key role in addressing social determinants of health. Success in this role requires strong clinical versatility, adaptability, and a commitment to hands-on, high-quality care in a resource-conscious environment.

Essential Job Functions
• Provide full-scope primary care, including preventive services, acute care, and chronic disease management for patients of all ages.
• Manage a consistent patient panel with a focus on continuity of care and long-term health outcomes.
• Diagnose and treat a wide range of conditions across the lifespan, including common adult and pediatric illnesses.
• Perform routine office-based procedures as appropriate (e.g., minor procedures, women’s health exams, basic dermatologic procedures).
• Deliver culturally competent health education tailored to varying levels of health literacy.
• Collaborate closely with nursing, care coordination, behavioral health, and social services teams to support whole-person care.
• Take ownership of follow-up care, including labs, referrals, hospital transitions, and chronic disease monitoring.
• Participate in preventive health initiatives, including screenings, immunizations, and community outreach efforts.
• Maintain accurate, timely documentation in the EHR.
• Contribute to Quality Improvement initiatives focused on access, clinical outcomes, and patient engagement.
• Supervise and support nurse practitioners, physician assistants, and clinical staff as needed.
• Participate in team meetings and contribute to a collaborative, mission-driven culture.
• Other duties as assigned.

Qualifications
Education and Certification
• MD from an accredited institution.
• Board Certified or Board Eligible in Family Medicine.
• Active New York State medical license and DEA registration.
• BLS certification (ACLS preferred).
Experience
• Experience in community health, outpatient primary care, or FQHC settings strongly preferred.
• Demonstrated commitment to serving underserved or high-need populations.
• Comfortable practicing broad-spectrum family medicine with limited subspecialty support.
