Key Responsibilities
- Provide comprehensive primary care services across the lifespan (pediatrics, adults, and geriatrics)
- Perform thorough patient assessments, including medical histories, physical exams, and preventive screenings
- Diagnose and manage acute and chronic conditions such as diabetes, hypertension, asthma, and other common illnesses
- Develop and implement individualized treatment plans in collaboration with patients and care teams
- Prescribe medications and therapies in accordance with New York State regulations
- Deliver patient education focused on disease prevention, health maintenance, and lifestyle modifications
- Maintain accurate, timely, and compliant electronic medical records (EMR) documentation
- Coordinate care with specialists, behavioral health providers, and community resources
- Participate in quality improvement initiatives and adhere to FQHC clinical standards and guidelines
Qualifications
- Active and unrestricted (LPN) Nurse Practitioner license in the State of New York
- DEA license (or eligibility to obtain)
- Experience in Family Medicine, Primary Care, or Community Health strongly preferred
- Prior FQHC or underserved population experience is a plus
- Strong clinical judgment and ability to work independently within a collaborative team
- Bilingual (Spanish or other languages) is a plus but not required
