Position Summary
A dedicated community health center in New York is seeking a compassionate, clinically skilled Family Nurse Practitioner (FNP) to join its primary care team under the direction of the Chief Medical Officer (CMO).
In this vital clinical role, you will manage daily primary medical care, preventative health screenings, and routine disease management for both adult and pediatric patient populations. Working within a Patient-Centered Medical Home (PCMH) and Chronic Care Management (CCM) framework, you will deliver comprehensive care, collaborate on quality improvement initiatives, and partner with community health agencies to ensure high-quality, patient-centered outcomes.
Key Responsibilities
- Primary Medical Care & Diagnostics: Provide comprehensive primary care and physical examinations for pediatric and adult patients; order, interpret, and evaluate diagnostic tests to formulate personalized treatment plans.
- Treatment & Chronic Care Management: Prescribe pharmacologic interventions, therapeutic regimens, and referrals; develop and implement individual patient care plans (PCMH and CCM) emphasizing goal setting and behavior change.
- Clinical Documentation & Compliance: Record physical findings, care plans, and clinical updates promptly within the Electronic Medical Record (EMR) system; ensure adherence to state, federal, and third-party billing/regulatory guidelines.
- Interdisciplinary & Community Coordination: Coordinate patient services with external medical specialists, community health programs, and social agencies to address barriers to care.
- Quality Improvement & Staff Education: Collaborate with the CMO to participate in chart audits, optimize documentation protocols, and develop in-service clinical training for health center staff.
- Infection Control & Safety: Adhere strictly to facility infection control protocols, personal protective equipment (PPE) requirements, and standard clinical safety guidelines.
Qualifications & Requirements
- Education: Master’s Degree in Nursing (MSN) conferred by an accredited college or university with Family Nurse Practitioner concentration.
- Licensure & Certifications:
- Active, unrestricted Family Nurse Practitioner (FNP) license in the State of New York.
- Current Healthcare Provider CPR / AED Certification.
- Maintenance of 24 Continuing Medical Education (CME) credits per 2-year credentialing cycle.
- Clinical Experience:
- Minimum of 1 to 3+ years of clinical experience as an NP in family medicine or primary care preferred.
- Experience treating individuals with intellectual and developmental delays or disabilities (IDD) is strongly preferred.
- Technical & Administrative Knowledge:
- Proficiency with Electronic Medical Record (EMR) software and standard computer applications (MS Office).
- Knowledge of Medicare, Medicaid, and third-party billing requirements.
- Language & Soft Skills: Excellent verbal/written communication and organizational skills; bilingual fluency in English/Spanish is preferred.

Compensation Note
- Salary Scale: Annual compensation is competitive and will be determined during the interview process based on clinical background, specialized population experience (IDD), and overall qualifications.
