Key Responsibilities:

  • Oversee and direct the delivery of patient care services, maintaining adherence to all regulatory standards.
  • Offer supervision, mentorship, and support to nursing staff to foster professional growth and enhance patient outcomes.
  • Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
  • Ensure accurate and prompt documentation of patient care and progress.
  • Conduct regular audits of patient records and care plans to guarantee quality assurance.
  • Act as a liaison between patients, families, and healthcare providers to ensure effective care coordination.
  • Engage in ongoing quality improvement initiatives and participate in professional development activities.

Required Qualifications:

  • Experience in a home care setting with at least two years of management or supervisory experience preferred.
  • Familiarity with professional standards and practices, Medicare Conditions of Participation, and regulatory requirements at federal, state, and local levels.

Required Certifications and Licensures:

  • Registered nurse with current licensure to practice nursing in the state of practice.
