As a Nurse Practitioner, you’ll deliver high-quality, comprehensive care to a diverse patient population — with a particular focus on Medicare adults — while working in a collaborative, team-based environment designed to support both you and your patients.
The Nurse Practitioner will provide longitudinal, relationship-driven primary care to adults in an outpatient setting. You’ll have the opportunity to practice autonomously while collaborating closely with physicians, care managers, medical assistants, and other members of the multidisciplinary care team.
You’ll focus on delivering equitable, preventive, and evidence-based care — emphasizing wellness, patient education, and continuity across the care journey.
Provide exceptional, evidence-based primary care to adult and geriatric patients.
Conduct comprehensive office visits for both routine and acute health concerns.
Perform annual wellness visits, health risk assessments, and thorough documentation using HCC and ICD-10 coding standards.
Coordinate care with specialists, hospitals, and other community resources.
Support population health initiatives, such as preventive screenings and outreach.
Educate patients and families on care plans, medications, and lifestyle management.
Participate in community health initiatives and occasional home visits.
Assist with triage and follow-up as needed.
Required:
Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or NP Certificate.
National certification in one of the following:
Family Nurse Practitioner (FNP)
Adult-Gerontology Primary Care NP (AGPCNP)
Adult NP or Gerontological NP
Active, unrestricted state license and DEA certification.
U.S. work authorization.
Strong commitment to patient-centered, team-based care.
Preferred:
Experience in primary care, family medicine, internal medicine, or geriatrics.
Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments.
Demonstrated ability to thrive in a collaborative, fast-paced clinical environment.
Pay Range: $90,000 – $190,000 (commensurate with experience and location)
