POSITION OVERVIEW
The Hospice Registered Nurse Case Manager will provide skilled nursing care and comprehensive case management services to hospice patients throughout Denton County.
The RN Case Manager will assess patient needs, manage pain and symptoms, coordinate the individualized plan of care, and provide education and emotional support to patients and families throughout the hospice journey.
KEY RESPONSIBILITIES

Provide compassionate skilled nursing care in accordance with each patient’s individualized plan of care.

Perform comprehensive patient assessments and continually monitor changes in condition.

Coordinate and manage the clinical care of assigned hospice patients.

Administer medications and treatments as prescribed by the physician.

Provide effective pain and symptom management.

Educate patients, families, and caregivers regarding medications, symptom management, safety, and the hospice philosophy of care.

Collaborate closely with physicians and the interdisciplinary hospice team to ensure coordinated, patient-centered care.

Communicate changes in patient condition promptly and appropriately.

Respond to urgent patient needs and provide crisis intervention when necessary.

Maintain complete, accurate, and timely clinical documentation in accordance with company, Medicare, and regulatory requirements.

Participate in the rotating on-call schedule.

Travel throughout the assigned Denton County territory.
IDEAL CANDIDATE

At least one year of hospice nursing experience.

Proficiency with electronic medical record systems.

Experience with Consolo is preferred.
REQUIRED QUALIFICATIONS

Current, unrestricted Texas Registered Nurse license.

Minimum of one year of hospice nursing experience.

Reliable transportation.

Valid automobile insurance.

Ability to travel throughout Denton County.

Ability to participate in an on-call rotation.

COMPENSATION
-
Base salary of $85,000–$90,000 annually, based on experience and qualifications

Mileage reimbursement at $0.65 per mile

Cell phone reimbursement of $60 per month
