LVN Patient Care Advocate - Hospice

Position Overview

A well-established hospice provider is seeking a clinically grounded, relationship-oriented LVN to step into a Patient Care Advocate role supporting hospice growth, referral-source relationships, patient/family education, and admissions coordination across Marin County.

This is a strong opportunity for a hospice clinician who enjoys patient education, referral-source interaction, and community-facing work, and who may be interested in expanding into a liaison, marketing, or hospice business development path while still using their clinical background every day.

What This Role Does

·        Partner with hospice liaisons and sales leadership to support referral-source development, census growth, and preference for hospice services in the local market.

·        Use clinical knowledge and empathetic communication to educate referral sources, patients, and families on hospice eligibility, services, philosophy, and care transitions.

·        Support admissions by gathering clinical information, helping coordinate next steps, and ensuring a responsive, service-oriented experience for patients, families, and referral partners.

·        Participate in community events, referral-source visits, and brand-awareness activity in support of the agency growth plan.

·        Serve as a knowledgeable hospice resource for referral partners, internal team members, patients, and families.

·        Document activity, referral-source communication, and marketing/admissions support in the CRM and other required systems.

Key Responsibilities

·        Collaborate with hospice liaisons to gather patient clinical information and support appropriate hospice admissions.

·        Conduct initial informational conversations with patients and families, explain hospice services, and help obtain required consents when appropriate.

·        Support referral-source relationships with hospitals, facilities, physician offices, senior living communities, and other community partners.

·        Help identify key accounts and communicate market feedback, opportunities, barriers, and competitive intelligence to sales and regional leadership.

·        Maintain accurate, timely documentation of outreach, referral activity, patient/family conversations, and related marketing reports.

·        Represent the hospice program professionally in the field, with a focus on service, responsiveness, and clinical credibility.

Required Qualifications

·        Current, unencumbered license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).

·        Minimum of 1 year of direct hospice experience required; 2-3 years of hospice experience preferred.

·        Strong hospice clinical knowledge and ability to explain hospice services clearly to patients, families, and referral sources.

·        Professional communication skills, follow-through, and comfort working in a field-based, relationship-driven role.

·        Prior customer service, liaison, marketing, admissions, or sales experience strongly preferred.

·        Valid driver's license, reliable transportation, and automobile insurance that meets state and organizational requirements.

·        Hospice LVN/LPN who understands hospice eligibility, admissions, family education, and referral-source expectations.

·        Comfortable moving between clinical conversations and relationship-building conversations.

·        Service-oriented, responsive, organized, and able to represent the hospice team professionally in the community.

·        Interested in career growth toward hospice liaison, admissions, marketing, or business development work.

·        Local to Marin County or familiar with the North Bay healthcare/referral-source environment.

Compensation and Benefits

·        $40-$45 per hour average pay range. Final compensation will be based on relevant experience, qualifications, and role scope.
