The Physical Therapist (PT) is responsible for evaluating, developing, and implementing individualized treatment plans to help patients improve mobility, reduce pain, restore function, and prevent disability. The Physical Therapist works collaboratively with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to deliver high-quality, patient-centered care across a variety of clinical settings. Key Responsibilities

• Patient Assessment & Evaluation
• Develop individualized treatment plans based on patients' functional needs, diagnoses, and goals.
• Establish measurable treatment objectives and adjust plans as patient conditions change.
• Document evaluations, progress notes, and discharge summaries accurately and timely.
• Design and implement therapeutic exercise programs to improve strength, mobility, balance, coordination, and endurance.
• Utilize manual therapy, therapeutic modalities, gait training, and functional activities to support recovery.
• Monitor patient progress and modify interventions as necessary.
• Educate patients on injury prevention, home exercise programs, and long-term wellness strategies.
• Provide education regarding diagnoses, treatment plans, rehabilitation expectations, and recovery timelines.
• Instruct patients and caregivers on proper body mechanics, adaptive equipment, and safety techniques.
• Encourage patient involvement in achieving therapy goals.
• Maintain compliance with regulatory, accreditation, and organizational standards.
• Adhere to documentation, billing, and coding requirements.
• Participate in department quality improvement initiatives and professional development activities.
• Maintain patient confidentiality in accordance with HIPAA guidelines.

Required Qualifications

• Graduate of an accredited Physical Therapy program.
• Doctor of Physical Therapy (DPT) degree preferred; Bachelor's or Master's degree accepted where applicable.
• Current Physical Therapist license in the applicable state.
• Current Basic Life Support (BLS) certification.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
