Home Health - RN, Registered Nurse

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Medford. This role presents an opportunity to make a meaningful impact by providing high-quality care to patients in the comfort of their homes.

Key Responsibilities:

Conduct comprehensive assessments of patients' health status and develop individualized care plans.
Provide nursing care and services to patients according to the plan of care and physician's orders.
Educate patients and their families on health maintenance and disease prevention.
Collaborate with other healthcare professionals to ensure coordinated and effective patient care.
Monitor and document patient progress and report changes to the healthcare team.

Qualifications:

Current and valid Registered Nurse (RN) license in the state of Massachusetts.
Bachelor's degree in Nursing or related field preferred.
Minimum of 1 year of experience in home health or related healthcare setting.
Strong clinical assessment and communication skills.
Compassionate, patient-focused, and able to work independently.
Valid driver's license and reliable transportation.
