We are seeking an experienced Full-time Home Health RN to join our team in beautiful Grants Pass. This rewarding role offers you the opportunity to provide personalized care and support to individuals who rely on your expertise and kindness.

Responsibilities:

Delivering in-home care to patients with a variety of medical conditions.
Developing and implementing individualized care plans in coordination with a multidisciplinary team.
Monitoring patient progress and making necessary adjustments to care plans.
Educating patients and their families on managing their health conditions and promoting overall wellness.
Documenting patient care and progress accurately and timely.
Collaborating with other healthcare professionals to ensure comprehensive patient care.

Requirements:

Current RN license in the state of Oregon.
Previous experience in home health care or a related field is preferred.
Strong assessment and clinical skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Valid driver's license and reliable transportation.
