We are currently seeking a skilled and compassionate Home Peritoneal Dialysis RN to join our team.
Job Summary: The Home Peritoneal Dialysis RN will be responsible for providing nursing care to patients undergoing peritoneal dialysis in their homes. The RN will work closely with patients and their families to ensure that they receive the best possible care and support. This position requires excellent communication and clinical skills, as well as the ability to work independently and as part of a team.
Responsibilities:
- Conduct patient assessments and develop care plans for patients undergoing peritoneal dialysis in their homes
- Provide patient education on peritoneal dialysis and related topics, including infection prevention, catheter care, and medication management
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes
- Collaborate with other healthcare providers, including physicians, pharmacists, and social workers, to ensure that patients receive comprehensive care
- Document patient care and progress in electronic medical records
- Participate in quality improvement initiatives to improve patient outcomes and satisfaction
- Maintain knowledge of current standards of care and best practices in peritoneal dialysis and related areas
Qualifications:
- Active in state RN license
- Minimum of 2 years of nursing experience, with at least 1 year of experience in peritoneal dialysis
- BSN degree preferred
- Strong clinical skills and knowledge of peritoneal dialysis and related topics
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical records and other healthcare technology
