We are recruiting an experienced Licensed Practical Nurse (LPN) Case Manager to support patients with chronic, complex, and serious illnesses. This is a full-time opportunity for an LPN who enjoys care coordination, patient education, home visits, palliative care, and case management work in the Jacksonville area.
Job Summary
The LPN Case Manager is responsible for coordinating care, educating patients and families, and serving as the main clinical point of contact for individuals with chronic or serious health conditions. This role combines nursing, care management, and patient advocacy, and is ideal for an LPN who wants to move into case management, home health, palliative care, or chronic care management.
Key Responsibilities
- Provide care coordination for a assigned caseload of patients.
- Conduct in-home visits or clinic visits to assess patient needs.
- Support patients with chronic care management and ongoing follow-up.
- Participate in palliative care or goals-of-care discussions when appropriate.
- Review medical records, labs, medications, and care plans.
- Educate patients and families about conditions, treatments, and resources.
- Document all interactions in the electronic health record (EHR).
- Coordinate with physicians, nurses, specialists, social workers, and home health teams.
- Assist with transitions of care from hospital to home or clinic.
- Provide emotional support and advocacy for patients with complex medical needs.
Qualifications
- Active Florida LPN license required.
- One to three years of experience in:
• Long-term care
• Primary care
• Home health nursing
• Hospice or palliative care (preferred)
• Case management or care coordination
- Strong skills in patient education and communication.
- Ability to work independently and manage a caseload.
- Reliable vehicle for home visits and field work.
