Director of Nursing (DON) – Skilled Home Health

The Director of Nursing (DON) provides clinical leadership and oversight for all home health services in compliance with Medicare Conditions of Participation, Florida AHCA regulations, and agency policies. The DON is responsible for clinical quality, staff supervision, patient outcomes, and successful management of state and federal surveys.

Key Responsibilities:

• Oversee clinical operations and ensure compliance with Medicare, CMS, AHCA, and accrediting body standards
• Lead preparation for, participation in, and follow-up from Medicare, state, and accreditation surveys
• Supervise and support nursing and clinical staff; ensure competency, training, and performance management
• Ensure accurate OASIS documentation, care planning, and quality assurance activities
• Monitor clinical outcomes, infection control, and patient safety initiatives
• Collaborate with administration, physicians, and interdisciplinary team members

Qualifications:

• Active Florida RN license (BSN preferred)
• Minimum 3–5 years certified home health experience, including supervisory or leadership role
• Strong working knowledge of Medicare regulations, CoPs, and survey readiness
• Experience as an in-field RN Case Manager
• Experience with EMRs (Axxess preferred)
