Key Responsibilities:
- Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
- Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs.
- Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality.
- Educate patients and their families about injury prevention, wellness strategies, and the recovery journey.
- Monitor and document patients' progress, adjusting treatment plans as necessary to achieve rehabilitation goals.
- Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.
- Maintain meticulous patient records and documentation, complying with regulatory standards.

- Experienced in educating patients and families on home safety protocols and proper medical equipment use
Required Certifications & Licensures
- Associate's degree from an accredited Physical Therapy program
- Active and valid license, registration, or permit to practice in the state of agency operation
- Current CPR certification, maintained continuously throughout employment in a clinical role
- Valid driver's license with the ability to travel across all business locations as needed
