Nurse Practitioner or Physician Assistant - Primary Care Functional Medicine

Responsibilities and Duties:
- Deliver direct patient care through office visits, telehealth, and occasional same-day appointments.
- Conduct comprehensive assessments, order and interpret labs/imaging, and diagnose both acute and chronic conditions.
- Develop personalized treatment plans that incorporate lifestyle changes, supplements, hormone therapies, and prescriptions.
- Educate and empower patients on preventive health and functional medicine principles.
- Collaborate with internal health coaches, nutritionists, mental health providers, and other clinical support staff.
- Ensure timely and compliant documentation of visits in the EHR (NextGen).
- Participate in team meetings, quality improvement initiatives, and training sessions.
- Contribute to a positive, team-oriented clinic environment.
Required Qualifications:
- Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain).
- Board certification in Family Medicine, Internal Medicine, or a relevant specialty.
- At least two years of clinical experience is preferred; experience in functional or integrative medicine is a plus.
- A strong interest in preventive, functional, and lifestyle medicine.
- Excellent communication and patient relationship skills.
- Comfortable working in a fast-paced, collaborative environment.
- Proficiency with electronic health records; experience with NextGen is a plus.
