Pharmacist-in-Charge (PIC) / Home Infusion Pharmacist

Position Summary

The PIC / Home Infusion Pharmacist is responsible for both day-to-day clinical pharmacy operations and overall regulatory compliance of the infusion pharmacy. This role serves as the accountable pharmacist to the Board of Pharmacy while also actively participating in patient care, sterile compounding oversight, and care coordination.

This is a hands-on leadership role, balancing compliance, clinical excellence, and operational execution to support safe and scalable growth.

 

Key Responsibilities

1. Regulatory & PIC Oversight

· Serve as the designated PIC and primary contact for the State Board of Pharmacy

· Ensure compliance with:

o USP <797>

o USP <800>

o State and federal pharmacy regulations

· Maintain pharmacy licenses, DEA compliance, and inspection readiness

· Lead accreditation readiness (ACHC, URAC) and audit response

· Develop, maintain, and enforce SOPs and policies

 

2. Clinical & Patient Care (Staff Pharmacist Role)

· Review, verify, and dispense prescriptions for infusion therapies (IVIG, antibiotics, biologics, TPN)

· Evaluate patient-specific data (labs, allergies, renal/hepatic function)

· Provide patient counseling and education

· Monitor therapy outcomes and collaborate with providers for adjustments

· Provide clinical support for intake and referral teams

 

3. Sterile Compounding & Pharmacy Operations

· Oversee sterile compounding and cleanroom operations

· Ensure accuracy, sterility, and compliance in all compounded preparations

· Supervise pharmacy technicians and compounding staff

· Maintain workflow efficiency from intake → dispense → delivery

· Ensure proper storage, labeling, and handling of medications

 

4. Care Coordination & Intake Support

· Work closely with intake team on:

o Benefits verification (VOB)

o Prior authorizations

o Clinical documentation review

· Coordinate with nurses, providers, and referral sources

· Support rapid therapy initiation (critical for competitive positioning)

· Act as clinical liaison for hospitals and physician offices

 

5. Inventory & Financial Stewardship

· Manage high-cost specialty drug inventory and cold chain logistics

· Monitor drug utilization, waste, and margin impact

· Navigate drug shortages and recommend alternatives

· Support cost control and formulary decisions

 

Qualifications

Education & Licensure

· PharmD or Bachelor of Pharmacy

· Active pharmacist license in state of practice

· Eligible to serve as PIC

 

Experience

· 2–5+ years pharmacy experience (home infusion, hospital, or specialty preferred)

· Strong sterile compounding experience required

· Prior leadership or PIC experience preferred

 

Skills & Competencies

· Strong understanding of pharmacy regulations and compliance

· Clinical expertise in infusion therapies

· Ability to balance hands-on work + leadership accountability

· Strong communication with providers, nurses, and internal teams

· Operational mindset with attention to speed and accuracy
