This role focuses on delivering restorative and rehabilitative physical therapy services both at the center and in participants' homes. Working alongside the interdisciplinary care team, the Physical Therapist promotes independence, safety, and functional restoration for participants.

Key Responsibilities:

  • Conduct assessments to determine therapy needs, goals, and care plans.
  • Provide and document therapy interventions as outlined in care plans.
  • Implement active and passive exercises, gait training, and functional ADL training.
  • Supervise therapy assistants in executing Functional Maintenance Programs (FMP).
  • Train staff, participants, and caregivers on therapeutic equipment use.

Qualifications:

  • Bachelor's degree in Physical Therapy (Master’s or Doctorate preferred).
  • Valid California Physical Therapy License or eligibility.
  • Experience working with elderly or frail populations preferred.
  • CPR certification preferred.

Compensation & Benefits:

  • Salary: $100,000-$125,000/year (total package includes base, bonus, and equity).
