Key Responsibilities:

  • Conduct thorough physical assessments and review medical histories to evaluate patients' conditions.
  • Develop and customize treatment plans tailored to each patient's specific needs and objectives.
  • Implement a range of therapeutic exercises and training programs to enhance patient recovery.
  • Provide education to patients and their families on injury prevention and wellness strategies.
  • Monitor and document patients' progress towards achieving their rehabilitation goals.
  • Collaborate with other healthcare providers to ensure comprehensive and holistic patient care.
  • Maintain accurate and up-to-date patient records in compliance with regulatory standards.

Qualifications:

  • Master’s degree in Physical Therapy.
  • Minimum of 1 year of experience in home health as a physical therapist.
  • Current licensure to practice as a physical therapist in the state of operation, adhering to professional standards.
  • Valid CPR certification required throughout employment in a clinical role.
  • Must possess a valid driver's license and the ability to travel to various business locations.
