Conduct thorough physical therapy evaluations in patient homes.
Develop and implement personalized treatment plans aimed at improving mobility, strength, safety, and independence for adult and geriatric clients.
Monitor progress, adjust interventions, and document clinical findings per Medicare and agency guidelines.
Provide caregiver/family education on home exercises, fall prevention, equipment use, and wellness strategies.
Coordinate with physicians, nursing staff, occupational therapists, and support services to ensure holistic care.
Participate in professional development — CEUs, team meetings, quality improvement initiatives.
Doctor of Physical Therapy (DPT) graduate from an accredited program (or foreign equivalent).
Active Maryland PT license (or eligible for licensure).
Experience in home health, geriatrics, orthopedics, or rehabilitation preferred; new graduates encouraged to apply.
Valid driver’s license and reliable transportation.
Excellent interpersonal skills, self-direction, and strong documentation and organizational abilities.
BLS certification (or willingness to obtain).
