Physical Therapist (PT) – Home Health

St. Mary’s County, MD (serving clients throughout St. Mary’s, Lexington Park, Leonardtown, and surrounding communities)

Key Responsibilities:

  • Conduct thorough physical therapy evaluations in patient homes.

  • Develop and implement personalized treatment plans aimed at improving mobility, strength, safety, and independence for adult and geriatric clients.

  • Monitor progress, adjust interventions, and document clinical findings per Medicare and agency guidelines.

  • Provide caregiver/family education on home exercises, fall prevention, equipment use, and wellness strategies.

  • Coordinate with physicians, nursing staff, occupational therapists, and support services to ensure holistic care.

  • Participate in professional development — CEUs, team meetings, quality improvement initiatives.

 

Qualifications:

  • Doctor of Physical Therapy (DPT) graduate from an accredited program (or foreign equivalent).

  • Active Maryland PT license (or eligible for licensure).

  • Experience in home health, geriatrics, orthopedics, or rehabilitation preferred; new graduates encouraged to apply.

  • Valid driver’s license and reliable transportation.

  • Excellent interpersonal skills, self-direction, and strong documentation and organizational abilities.

  • BLS certification (or willingness to obtain).
