Initiate and implement personalized physical therapy programs based on physician orders
Maintain detailed evaluations, progress notes, and discharge summaries
Collaborate with nursing and rehab staff to enhance residents' functional outcomes
Contribute to interdisciplinary care planning meetings
Maintain treatment documentation, track patient statistics, and request necessary supplies
Participate in or lead staff training and education
Provide leadership support in the absence of the Director of Physical Therapy
Graduate of an accredited Physical Therapy program
Active NY State Physical Therapy license
Strong interest in working with geriatric, subacute, and adult patient populations
Previous experience preferred (but not requiredânew grads are encouraged to apply!)
Leadership qualities and ability to direct support staff when needed
