Seeking an experienced, licensed leader to serve as the Manager of Social Work & Care Management at a premier regional medical center in Everett, WA. This role is designed for a clinical leader who understands the complex intersection of social determinants of health, clinical case management, and hospital operations.

You will lead a multidisciplinary team of Social Workers and Case Managers, ensuring that patient care is coordinated, transitions are seamless, and the facility maintains the highest standards of clinical and financial integrity. This is a high-impact leadership role that requires a balance of clinical expertise (LICSW) and operational excellence.

Key Responsibilities

Operational Leadership & Strategy

  • Departmental Oversight: Direct the daily operations of the Social Work and Care Management departments, including discharge planning, utilization management, and pre-admission screening.

  • Standardization: Establish and monitor clinical standards and "best practice" protocols to optimize patient throughput and reduce avoidable delays in care.

  • Financial Stewardship: Ensure the financial integrity of the organization through effective utilization review and strategic resource allocation.

Clinical & Social Work Advocacy

  • Complex Case Management: Act as the subject matter expert for high-complexity cases involving social work intervention, legal/ethical dilemmas, and vulnerable populations.

  • Continuum of Care: Drive the strategy for safe and effective discharge planning, maintaining strong partnerships with SNFs, community agencies, and home health providers.

  • Patient & Family Liaison: Serve as a senior-level mediator between patients, families, and the medical team to ensure a "whole-person" approach to care.

Team Development & Culture

  • Mentorship: Supervise and empower a team of clinical social workers and coordinators, fostering an environment of continuous professional development.

  • Collaboration: Work closely with nursing leadership, physicians, and executive stakeholders to align care management goals with broader hospital initiatives.

Required Qualifications

  • Education: Master’s Degree in Social Work (MSW) from an accredited school.

  • Licensure: Active Washington Clinical Independent Social Worker License (LICSW).

  • Experience: Minimum of 6 years of experience in Case Management, specifically within an acute care hospital setting.

  • Leadership: Minimum of 2 years of Managerial or Director-level experience, preferably managing Social Work or Care Management teams in a large hospital environment.

  • Environment: A supportive, patient-focused culture that values the unique perspective of Social Work in the medical field.
