Physical Therapist

Qualifications

Required:

  • Master’s degree or higher in Physical Therapy
  • 1+ years of experience as a Physical Therapist
  • Valid Licenses/Certifications:
    • PT License in California (CA)
    • American Heart Association Basic Life Support (AHA BLS)
    • Proof of a valid California driver’s license, personal transportation, a good driving record, and auto insurance as required by state law
  • This is an on-site role—are you willing to commute to the Greater LA area every day?

Preferred:

  • 1+ years of experience in any of the following:
    • Value-Based Care
    • PACE Experience
    • Working with frail adults

Overview

As a Physical Therapist for Our Award Winning Client, you will play a vital role in developing personalized care plans and delivering life-changing therapeutic care to older adults. Your expertise will help participants regain mobility and independence, while also educating them and their families for continued progress at home.

Key Responsibilities

  • Participate in interdisciplinary team (IDT) meetings to complete assessments, develop individualized care plans, and review service determination requests.
  • Evaluate mobility levels, physical conditions, and functional abilities to create tailored therapy plans.
  • Deliver therapeutic exercises, gait training, and ambulation support to help participants achieve their goals.
  • Develop engaging individual and group exercise programs focused on maintenance therapy and rehabilitation.
  • Provide physical therapy services aligned with physician orders and participant care plans, documenting interventions accurately.
  • Conduct thorough assessments to identify priority areas and recommend follow-ups as needed.
  • Recommend referrals for participants who may benefit from additional services or resources.
  • Offer education to participants and their families, equipping them with the tools and knowledge needed for successful rehabilitation.
  • Assist with tracking, managing, and maintaining therapy equipment to ensure participant safety and program efficiency.
  • Work closely with staff in nursing homes, skilled nursing facilities (SNFs), and assisted living settings to oversee rehabilitation needs for participants.
  • Provide approvals for care plans in nursing home settings on behalf of the IDT.
  • Conduct home assessments and deliver in-home treatments to meet participants where they are most comfortable.

Schedule & Shift Details

  • Monday - Friday, 8:00 AM - 5:00 PM (On-site)

Travel

  • Primarily on-site at the facility
  • Occasional travel to participants’ homes, SNFs, or healthcare facilities

Compensation & Benefits

  • Salary: $100,000 - $120,000/year (dependent on experience)
  • Mileage reimbursement for travel

Additional Notes

  • This is a 100% on-site Physical Therapy position.
  • Experience working with elderly and frail adults is highly preferred.
  • Experience with Value-Based Care or PACE is a plus.
  • Strong documentation skills are beneficial.
  • Must be willing to commute daily to the Greater Los Angeles area.
