    • Primary Care
    • Value-Based Care
  • Valid Nurse Practitioner (NP) license in the state of Tennessee (TN)
  • Home visits are a major component of this job. Are you okay visiting patients in their homes? (yes)

Preferred:

  • HCC Coding experience (yes)

The licensed Nurse Practitioner will lead the medical aspects of team-based care for individuals with serious mental illness. This position is responsible for documenting and supporting the medical needs of individuals through whole-person clinical assessments that identify and address care gaps. This role also involves supporting care coordination with the individual's medical and behavioral health providers and providing clinical guidance to Guides (peers) and Community Resources Guides (social workers).

Responsibilities:

  • Perform whole-person clinical assessments, including:

    • Gathering information from other care providers
    • Assessing both behavioral and physical health through history and physical examination
    • Capturing undiagnosed conditions and ordering additional studies as needed
    • Organizing information to support disability applications (if applicable)
    • Conducting visits in the field or individual homes as required
    • Making individualized recommendations or referrals to programs or outside providers
  • Lead the medical component of team-based care, including:

    • Training and advising firsthand Guides and Community Resources Guides on clinical issues
    • Supporting care coordination for the Guide Team
    • Managing care and identifying actionable care gaps
    • Performing medication reconciliation post-discharge and as needed
    • Communicating and coordinating with other care providers (CMHCs, FQHCs, hospitals)

  • Intermediate computer skills, proficiency with Microsoft Office Suite
  • A valid driver’s license, private auto insurance, and insurability under firsthand coverage

Experience Required:

  • 5+ years of clinical experience as a Nurse Practitioner, ideally in a primary care setting
  • Experience working with underserved populations facing challenges like behavioral health or SUD conditions
  • Experience in developing and delivering educational programs for nurses and affiliated medical staff
  • Care management and coordination expertise

Compensation:

  • $142.5K base salary

  • 10% bonus potential

  • Incentive: At 36 months, a guaranteed lump sum payment of $30K plus a $10K base salary adjustment

  • $2K allowance for continuing education and recertification

  • Normal Monday to Friday schedule, with rare weekend consultative calls (expected to be light or non-existent)

Must Haves:

  • 5+ years of clinical care experience as a Nurse Practitioner, ideally in primary care
  • Strong focus on primary care
  • Must be from and have connections in the local community

Preferred:

  • HCC Coding experience
