Qualifications

Required:

• Associate degree or higher
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)
• ANY of the following valid licenses/certifications:
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • American Heart Association Basic Life Support (AHA BLS)
• Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all three)?

Preferred:

• Please provide your LPN License Number.

Overview

Our Award Winning Client is seeking a compassionate Home Health LPN to join their extraordinary care team in Milford, PA. The ideal candidate will have an Associate degree, a valid LPN license, and at least two years of LPN experience. Responsibilities include collaborating to provide patient care plans, supporting patients in the local area, and demonstrating proficiency with technology tools.

Key Responsibilities

• Collaborate to provide management and delivery of patient care plans
• Support patients in Milford, PA, and surrounding areas
• Meet critical deadlines for charting
• Self-manage time and schedule efficiently
• Demonstrate experience with tablets, mobile phones, and EMR software

Schedule and Shift Details

• Schedule: Monday-Friday, 8 AM - 5 PM with one weekend per month (Saturday & Sunday, 8 AM - 5 PM) Two weekdays off to compensate for weekend hours.

Travel

• Occasional travel may be required within the local area

Home Health LPN – $10,000 Bonus

• Location: Milford, PA
• Territory/Driving Expectation: Pike County
• Preferred Experience: Home Health/Hospice, Acute Care, LTC, SNF
• Not Preferred: N/A
• Pay Rate/Salary: $28-$29/hour with a $10,000 FYS bonus
• Schedule: Monday-Friday, 8 AM - 5 PM with one weekend per month (Saturday & Sunday, 8 AM - 5 PM) Two weekdays off to compensate for weekend hours.
