Job Description:Our Award Winning Client is seeking to add to their exceptional care team. Driven by a belief that superior care is best delivered in a team-based environment, their home health physical therapists collaborate to manage and deliver patient care plans. This position supports patients in Montoursville and surrounding areas.

Qualifications Required:

• Bachelor’s degree or higher in Physical Therapy from an accredited college
• Healthcare experience (1+ years)
• 1+ years of experience in any of the following:
• Electronic Medical Record (EMR)
• Practice Partner EMR
• Other EMRs
• Valid Physical Therapist license in Pennsylvania (PA)
• Valid Regular Driver License (DL)
• Please provide your PT license number

Preferred Qualifications:

• Experience in home health, hospital, LTC, or SNF settings

Overview of Responsibilities:

• Deliver patient care plans as part of a collaborative team
• Provide care management and charting using state-of-the-art technology
• Commitment to meeting deadlines for charting and patient care
• Maintain daily travel between patient locations (approximately 20% of time)
• Self-manage time and schedule effectively
• Utilize EMR software, tablets, and mobile phones proficiently

Compensation:This is a PPV (Pay-Per-Visit) position with rates varying based on type of service and experience level. PPV ranges between $52-$60 per visit plus a $10,000 first-year success bonus.
