Qualifications

Required:

• High School Diploma or GED
• 2+ years of experience in all of the following:
  • Medical Scheduling
  • Electronic Medical Records (EMR)
• 1+ years of experience in any of the following:
  • Frail Adult Experience
  • Geriatric Care
  • Geriatric/LTC
• Are you bilingual in Spanish?

Preferred:

• 1+ years of experience in any of the following:
  • Value-Based Care
  • PACE

Overview

Our Award Winning Client is seeking a PACE Scheduler to coordinate participant appointments and ensure seamless care delivery for older adults. This role is essential in managing scheduling, communication, documentation, and collaboration with key teams to support the well-being of the seniors we serve.

Key Responsibilities

• Coordinate and schedule participant appointments
• Facilitate external appointments
• Organize new participant orientation & initial assessments
• Optimize scheduling across care settings
• Collaborate with key teams to ensure continuity of care
• Serve as a liaison between key departments
• Communicate with participants and provide necessary follow-ups
• Manage scheduling conflicts and urgent requests
• Maintain accurate documentation in EMR systems

Schedule and Shift Details

• Monday - Friday, 8:00 AM - 5:00 PM (On-site)
