Field Nurse Supervisor

Compensation: $115K–$125K per year

Job Type: Full-time, Onsite/Field-Based (Monday through Friday schedule)

Location: Riverdale, NY

Our client is seeking a dedicated and experienced Field Nurse Supervisor to oversee field-based nursing and paraprofessional staff within a Licensed Home Care Services Agency (LHCSA). This role offers the opportunity to lead a compassionate care team that delivers high-quality, person-centered home health services to older adults, supporting their independence and well-being in the community.

Key Responsibilities

• Provide leadership, direction, and supervision for paraprofessional staff including home health aides, personal care aides, companions, and nurses.
• Conduct field visits to monitor care delivery and provide professional supervision and support.
• Facilitate on-the-job training, skills validation, and competency assessments for staff.
• Collaborate with administrative and clinical teams to ensure compliance with agency standards and state regulations.
• Assist with classroom training sessions and continuing education initiatives.
• Uphold safety, clinical quality, and documentation standards across all field operations.

Qualifications

• Active New York State Registered Professional Nurse (RN) license.
• Bachelor’s degree in Nursing or related health/human services field preferred.
• Minimum of 1 year of nursing experience in an acute care setting.
• Prior home care experience strongly preferred.
• In-depth understanding of the scope of practice for professional and paraprofessional healthcare staff.
• Strong communication, organizational, and documentation skills.
• Ability to interpret policies, procedures, and safety protocols effectively.
• Proficiency in performing medication calculations and clinical measurements accurately.
