Your Role with the Company:

As the senior Physical Therapist, you will provide physician prescribed physical therapy for patients while completing a timely, comprehensive assessment on patients upon first visit to home. You will be required to work to develop an appropriate plan of care with goals that have been mutually agreed upon with patient/family based on assessment and physician’s orders.

Location:

This client has multiple locations throughout Shiawassee, Saginaw, and Genesee Counties that are close to the freeway and easy to access! You will be requested to go to patient homes to assist with their physical therapy session.

Experience Profile:

• Graduate from an approved school of Physical Therapy and licensed to practice in the State of Michigan.
• 2 years of previous Home Care experience preferred.
• Knowledge of Home Health regulations under Medicare and other Third party reimbursement agencies.
