LVN, Licensed Vocational Nurse (Per Diem)

We are seeking dedicated and compassionate Licensed Vocational Nurses (LVNs) to join our dynamic home health team in West Los Angeles. This opportunity is available for both Per Diem and Full-time positions. As an LVN, you will play a critical role in providing exceptional care to patients in the comfort of their homes.

Role Responsibilities:

Conduct home health visits and follow-up care for patients with various medical needs.
Deliver skilled nursing care and prescribed medical treatments in accordance with the Nurse Practice Act of California.
Collaborate with a supervising Nurse Practitioner or Physician to ensure quality patient care.
Perform basic medical care tasks such as changing bandages, administering medications, and collecting specimens.
Assist patients with daily activities including bathing, feeding, and dressing to ensure their comfort and safety.
Monitor patients' conditions and report any changes to the supervising nurse.
Support Registered Nurses in educating patients and their families about health management and care plans.

Qualifications:

Active LVN license in the state of California.
Minimum of one year of clinical experience is required.
Valid driver’s license with a clean driving record.
