POSITION SUMMARY
Cedarstone Harbor is seeking dependable, compassionate Certified Nursing Assistants to provide respectful, hands-on care to residents in our skilled nursing, rehabilitation, and memory-care neighborhoods. The CNA works closely with nurses and the interdisciplinary team to support activities of daily living, observe changes in resident condition, document care, and create a safe, dignified resident experience.
ESSENTIAL RESPONSIBILITIES
Assist residents with bathing, dressing, grooming, toileting, mobility, transfers, repositioning, eating, and other activities of daily living.
Take and accurately record vital signs, weights, intake/output, and other assigned observations within the required timeframes.
Recognize and promptly report changes in a resident’s physical condition, behavior, pain, skin integrity, appetite, or level of function to the licensed nurse.
Use proper transfer techniques, gait belts, mechanical lifts, and infection-prevention practices according to training and facility policy.
Answer call lights and resident requests promptly while communicating with patience, empathy, and professionalism.
Support individualized care plans, restorative nursing programs, fall-prevention practices, and dementia-care approaches.
Complete electronic documentation accurately and protect resident privacy in accordance with HIPAA and facility policy.
Collaborate with nurses, dietary, therapy, activities, social services, and other team members to provide coordinated care.
Maintain clean, organized resident areas and follow safety, emergency, abuse-prevention, and mandatory-reporting requirements.
Participate in shift reports, team huddles, required training, competency checks, and performance-improvement activities.

MINIMUM QUALIFICATIONS Active CNA certification in Wisconsin and good standing on the Wisconsin Nurse Aide Registry. Ability to read, write, speak, and understand English sufficiently to communicate and document resident care; bilingual skills are welcomed. Ability to work assigned weekends and holidays based on the facility schedule. Reliable attendance, professional conduct, and a resident-first approach. Successful completion of background, reference, health, and credential requirements. |

SCHEDULE, PAY & BENEFITS Schedule: Full-time and part-time openings on day, PM, and overnight shifts; rotating weekends and holidays may be required. Compensation: $20.50–$25.00 per hour, plus applicable evening, overnight, weekend, and pickup differentials. PREFERRED EXPERIENCE & REPORTING Six months or more of CNA experience in skilled nursing, rehabilitation, assisted living, memory care, or hospital-based care is preferred but not required. Experience with point-of-care electronic documentation, restorative nursing, dementia support, or mechanical lifts is helpful. The CNA reports to the charge nurse or nurse manager and works closely with the Director of Nursing, therapy, dietary, activities, and social services. |
PHYSICAL & WORK REQUIREMENTS
The role regularly requires standing, walking, bending, reaching, pushing, pulling, and assisting with resident transfers. Team members must be able to perform the essential functions of the position, with or without reasonable accommodation, and follow all required personal-protective-equipment and infection-control procedures. Work may involve exposure to bodily fluids, infectious conditions, emotionally difficult situations, and a fast-paced care environment.
