Your Core Responsibilities:
· Deliver Compassionate, Patient-Centered Care: Take charge of a group of patients, assessing their health needs, developing personalized care plans, and ensuring they receive the support they need to thrive.
· Lead and Inspire: Drive patient care by coordinating a team approach to each care plan, directing others, and taking ownership of outcomes for your patients.
· Advocate for Patients: Serve as a trusted guide for patients and families, helping them understand care plans, listening to their concerns, and supporting them at every step of their journey.

Additional Responsibilities:
· Maintain Documentation & Ensure Compliance: Accurately document patient interactions and uphold all regulatory standards to ensure high-quality, compliant care.
· Mentor and Develop Your Team: Share your knowledge, support the growth of new team members, and foster a collaborative work environment that brings out everyone’s best.
· Respond in Critical Situations: Be prepared to act swiftly in emergencies, drawing on your training to provide effective care and support when it’s needed most.
Minimum Qualifications:
· Education: Graduate from an accredited school of professional nursing.
· Licensure: Licensed as a Registered Nurse in the State of Missouri. OR willing to obtain one before employment
· Experience: At least one year of nursing experience preferably in long term care, home care, hospice, skilled nursing facility, medsurg
