Join our team as a dedicated Physical Therapist (PT) on a per diem basis, where your skills and commitment will significantly impact patients' lives. We are seeking a compassionate and skilled professional to provide therapeutic care aimed at enhancing mobility, reducing pain, and restoring function through tailored treatment plans.

Key Responsibilities:

  • Review resident information to evaluate its impact on assessments and treatments.
  • Conduct comprehensive functional assessments using standardized procedures.
  • Develop and implement personalized treatment plans with realistic functional goals.
  • Ensure compliance with evaluation, treatment, and documentation standards.
  • Supervise and instruct therapy assistants during treatment procedures.
  • Document treatment progress and revise plans as necessary.
  • Educate residents and caregivers on exercises and procedures for home continuation.
  • Collaborate with nursing staff and families to maximize residents’ functional abilities.
  • Participate in team meetings and quality improvement initiatives.
  • Stay informed of community resources and assist in referral processes post-discharge.

Qualifications:

  • At least one (1) year of experience as a PT, Home Health experience preferred
  • Valid California PT license required
  • Current CPR certification required 
  • Current California driver’s license, good driving record, and reliable transportation
  • Located in Southern California

Physical Requirements:

  • Ability to stand for prolonged periods.
  • Capability to push or pull with a force of up to 35 lbs.
  • Ability to lift up to 50 lbs independently and over 50 lbs with assistance.
  • Capacity to stoop, reach, and maintain balance on various surfaces.
