The Home Health Clinical Manager oversees the delivery of high-quality patient care in a home health setting. This role involves managing clinical staff, ensuring compliance with regulations, and coordinating care to meet patient needs. The Clinical Manager acts as a leader, mentor, and liaison between staff, patients, and administration.

Key Responsibilities

  • Staff Supervision: Manage and support a team of nurses, therapists, and aides, including hiring, training, scheduling, and performance evaluations.

  • Care Coordination: Oversee patient care plans, ensuring individualized, effective, and timely interventions in collaboration with physicians and other healthcare providers.

  • Compliance: Ensure adherence to federal, state, and local regulations, as well as agency policies, including Medicare/Medicaid standards and Joint Commission accreditation.

  • Quality Assurance: Monitor clinical outcomes, conduct audits, and implement quality improvement initiatives to enhance care delivery.

  • Patient Advocacy: Address patient and family concerns, ensuring satisfaction and resolving issues promptly.

  • Documentation: Review and approve clinical documentation for accuracy, completeness, and compliance with regulatory requirements.

  • Budget Oversight: Assist in managing departmental budgets, optimizing resources while maintaining care standards.

  • Education and Training: Provide ongoing staff education on clinical best practices, new regulations, and agency protocols.

  • Interdisciplinary Collaboration: Work with administrative staff, case managers, and external partners to streamline operations and improve patient outcomes.

Qualifications

  • Education: Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing or Healthcare Administration preferred.

  • Licensure: Active Registered Nurse (RN) license in the state of practice.

  • Experience: Minimum of 2-3 years of clinical experience in home health or related field, supervisory or management role.

  • Skills:

    • Strong leadership and team management abilities.

    • Excellent communication and interpersonal skills.

    • Knowledge of home health regulations and reimbursement processes (e.g., Medicare, Medicaid).

    • Proficiency in electronic health records (EHR) and clinical software.

    • Problem-solving and decision-making skills.

  • Certifications: Home Health Care Certification (preferred); Basic Life Support (BLS) certification required.
