Nurse Practitioner (APRN) / Physician Assistant (PA)

Now, we’re looking for a dedicated and compassionate Nurse Practitioner (APRN) or Physician Assistant (PA) to join our Adult Medicine team. If you’re passionate about health equity, collaborative care, and improving lives every day — we want to hear from you.

What You’ll Do

As an APRN or PA you will:

  • Deliver comprehensive primary care with a focus on prevention, wellness, and chronic disease management.

  • Collaborate closely with physicians and multidisciplinary teams to ensure holistic, culturally competent care.

  • Provide diagnostic assessments, treatment planning, and patient education.

  • Maintain thorough, timely, and accurate documentation in our electronic health record (EHR) system.

  • Refer patients for specialist care as needed and follow up to ensure continuity.

  • Champion quality standards and help shape better outcomes for our community.

What You’ll Bring

  • Graduation from an accredited Nurse Practitioner or Physician Assistant program.

  • Current license to practice in the State of Connecticut.

  • DEA and state narcotic registration (or eligibility to obtain).

  • 1–2 years of experience in ambulatory care or urgent care preferred.

  • Clinical expertise in adult medicine, with strong decision-making and diagnostic skills.

  • Passion for serving a diverse, often underserved population with cultural sensitivity.

  • Excellent communication and documentation skills.

  • Proficiency in using EHR systems.
