We are seeking a dedicated and skilled Physical Therapist to join our team in providing exceptional home health care services on a part-time basis. This role is ideal for a compassionate individual committed to enhancing the quality of life for patients with various physical challenges.

Role Overview:

As a Home Health Physical Therapist, you will work collaboratively as part of an interdisciplinary team to assess, plan, and implement personalized treatment strategies aimed at restoring patients' mobility, managing pain, and improving overall physical function. You will utilize a diverse range of therapeutic techniques, including manual therapy, exercise regimens, and advanced modalities, all tailored to meet the unique needs of each patient.

Key Responsibilities:

  • Evaluate patient information and determine the impact on treatment plans.
  • Conduct comprehensive functional assessments using standardized methods.
  • Develop and execute individualized treatment plans with clear, achievable goals.
  • Ensure timely and accurate documentation of all evaluation and treatment activities.
  • Provide guidance and supervision to therapy assistants during patient care.
  • Document progress and adjust treatment plans as necessary to achieve desired outcomes.
  • Educate patients and caregivers on home exercise programs and ongoing care strategies.
  • Collaborate with nursing staff and patient families to optimize functional outcomes.
  • Participate in team meetings, continuing education, and quality improvement initiatives.
  • Adhere to professional and regulatory standards while maintaining a high level of clinical competency.

Qualifications:

  • Bachelor’s or Master’s degree in Physical Therapy.
  • Valid state licensure as a Physical Therapist.
  • Experience in a home health or similar setting is preferred.
  • Strong communication and interpersonal skills.
  • Proficiency in applying therapeutic principles and techniques.
  • Ability to perform physical tasks required for patient care, including lifting up to 50 lbs.
