This pivotal role involves enhancing the quality of patient care in a home health setting, ensuring that every patient receives compassionate, personalized attention.

Key Responsibilities:

  • Oversee and manage the delivery of patient care services in compliance with regulatory standards.
  • Provide leadership, guidance, and support to nursing staff, promoting professional development and improved patient outcomes.
  • Collaborate with interdisciplinary teams to create and implement tailored patient care plans.
  • Ensure timely and accurate documentation of patient care activities and progress.
  • Perform regular audits of patient records and care plans to maintain quality assurance.
  • Act as a liaison among patients, families, and healthcare providers to ensure effective care coordination.
  • Participate in ongoing quality improvement projects and engage in professional development opportunities.

Required Qualifications:

  • Minimum of two years of management or supervisory experience in a home care setting preferred.
  • Knowledge of professional standards, Medicare Conditions of Participation, and compliance with federal, state, and local regulations.

Required Certifications and Licensures:

  • Registered nurse with current licensure to practice nursing in the state of practice.
