Hospice LPN – Full-time

Qualifications

Required:

• Associate degree or higher
• 2+ years of hospice experience
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• Valid Licensed Practical Nurse (LPN) in the state of Illinois (IL)
• Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all three)?
• Are you okay with the required travel for this position?

Preferred:

• Please provide your LPN License Number (Required)

Key Responsibilities

• Collaborate to provide management and delivery of patient care plans
• Support patients in Shiloh/Fairview Heights and surrounding areas
• Meet critical deadlines for charting
• Self-manage time and schedule
• Drive to/from patient locations

Schedule and Shift Details

• Work schedule and shift expectations will be provided during onboarding

Travel

• Occasional local travel within the specified territory
