We are seeking a dedicated and experienced Registered Nurse to join our team as a Patient Care Manager in Home Health. This role is pivotal in ensuring high-quality care and support for our patients. The ideal candidate will possess strong leadership skills and a passion for improving patient outcomes.
Key Responsibilities:
- Oversee and manage patient care services in a home health setting.
- Coordinate with multidisciplinary teams to ensure comprehensive patient care.
- Develop and implement individualized patient care plans.
- Monitor patient progress and adjust care plans as necessary.
- Ensure compliance with healthcare regulations and standards.
- Provide education and support to patients and their families.
- Supervise and mentor nursing staff, promoting professional development.
Qualifications:
- Current Registered Nurse (RN) license.
- 1 year experience as a Registered Nurse.
- Minimum of 2 years of experience in home health or related field.
- Proven leadership and management experience.
- Excellent communication and interpersonal skills.
- Strong critical thinking and problem-solving abilities.
- Ability to work independently and effectively manage time.
