Suffolk Position Summary:A qualified professional who provides physical therapy services to patients as prescribed by the attending physician under the direct (but not on-site) supervision of a licensed Physical Therapist.
Responsibilities:
Treat patients to relieve pain and/or develop or restore function to reach the highest level of independence possible.
Instruct patients and their families in in transfer techniques, home exercise program and activities of daily living, use of prosthetic, orthotic and assistive devices
Write clinical progress notes on the date of the visit for inclusion into the patient’s permanent record.
Perform an on-site, joint visit with the assigned Physical Therapist initially, and communicate regularly for instruction in changes in the patient’s program.
Communicate regularly with the case manager and other health care professionals assigned to a case.
Requirements:
Graduate of an accredited Physical Therapist Assistant program.
Licensed by and currently registered with the New York State Education Department.
One year of practical experience as a licensed physical therapist assistant.
