This role focuses on providing restorative and rehabilitative physical therapy services at both the center and in participants' homes. Collaborating with the interdisciplinary care team, the Physical Therapist helps maximize independence, safety, and functional restoration for participants.

Key Responsibilities:

  • Conduct assessments to determine therapy needs, goals, and care plans.
  • Implement and document therapy interventions per care plans.
  • Provide exercise, gait training, functional ADL training, and therapeutic treatments.
  • Supervise therapy assistants in executing Functional Maintenance Programs (FMP).
  • Train staff, participants, and caregivers on therapeutic equipment use.

Qualifications:

  • Valid California Physical Therapy License or eligibility.
  • Bachelor’s in Physical Therapy (Master’s or Doctorate preferred).
  • Experience with elderly or frail populations preferred.
  • CPR certification preferred.

Compensation & Benefits:

  • Salary: $99,309 - $119,171/year (including base, bonus, and equity).
