Job Summary:

The Home Health Physical Therapist (Weekend Baylor) provides skilled physical therapy services to patients in their homes, helping them regain mobility, manage pain, and improve functional independence. The Baylor schedule typically involves working two 12-hour weekend shifts (e.g., Saturday and Sunday, or Friday–Sunday in some cases), equivalent to a full-time role with benefits, allowing weekdays off. The PT assesses patient needs, develops treatment plans, and collaborates with healthcare teams to support recovery in a home setting.

Key Responsibilities:

  • Patient Assessment: Evaluate patients’ physical conditions, functional limitations, and home environments to determine therapy needs, considering factors like mobility, safety, and medical history.
  • Treatment Planning: Create individualized care plans to improve strength, balance, coordination, and pain management, tailored to patients’ home settings and rehabilitation goals.
  • Therapeutic Interventions: Deliver treatments including therapeutic exercises, manual therapy, gait training, balance activities, and modalities (e.g., ultrasound, TENS), adapting to the home environment.
  • Patient and Caregiver Education: Teach patients and families home exercise programs, safe mobility techniques, and strategies to prevent falls or re-injury.
  • Documentation: Maintain accurate records of evaluations, progress notes, and discharge summaries in compliance with Medicare, Medicaid, and agency regulations.
  • Collaboration: Coordinate with physicians, home health nurses, occupational therapists, and case managers to ensure comprehensive patient care.
  • Home Safety Assessments: Recommend adaptive equipment (e.g., walkers, grab bars) and home modifications to enhance safety and accessibility.
  • Travel: Drive to patients’ homes within a designated service area, managing a caseload of 5–7 patients per day, depending on visit duration and travel time.

Qualifications include:

  • Master’s degree in physical therapy
  • 1 year of home health experience as a physical therapist
  • Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations
