The Home Health Clinical Manager oversees the delivery of high-quality patient care in a home health setting. This role involves managing clinical staff, ensuring compliance with regulations, and coordinating care to meet patient needs. The Clinical Manager acts as a leader, mentor, and liaison between staff, patients, and administration.
Staff Supervision: Manage and support a team of nurses, therapists, and aides, including hiring, training, scheduling, and performance evaluations.
Care Coordination: Oversee patient care plans, ensuring individualized, effective, and timely interventions in collaboration with physicians and other healthcare providers.
Compliance: Ensure adherence to federal, state, and local regulations, as well as agency policies, including Medicare/Medicaid standards and Joint Commission accreditation.
Quality Assurance: Monitor clinical outcomes, conduct audits, and implement quality improvement initiatives to enhance care delivery.
Patient Advocacy: Address patient and family concerns, ensuring satisfaction and resolving issues promptly.
Documentation: Review and approve clinical documentation for accuracy, completeness, and compliance with regulatory requirements.
Budget Oversight: Assist in managing departmental budgets, optimizing resources while maintaining care standards.
Education and Training: Provide ongoing staff education on clinical best practices, new regulations, and agency protocols.
Interdisciplinary Collaboration: Work with administrative staff, case managers, and external partners to streamline operations and improve patient outcomes.
Education: Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing or Healthcare Administration preferred.
Licensure: Active Registered Nurse (RN) license in the state of practice.
Experience: Minimum of 2-3 years of clinical experience in home health or related field, supervisory or management role.
Skills:
Strong leadership and team management abilities.
Excellent communication and interpersonal skills.
Knowledge of home health regulations and reimbursement processes (e.g., Medicare, Medicaid).
Proficiency in electronic health records (EHR) and clinical software.
Problem-solving and decision-making skills.
Certifications: Home Health Care Certification (preferred); Basic Life Support (BLS) certification required.
