Key Responsibilities:

  • Administer quality therapeutic care to facilitate patients' recovery, focusing on orthopedic and neurological conditions.
  • Implement and adapt treatment plans through manual and non-manual exercises, ensuring patient progress towards their functional goals.
  • Support patients in performing ambulatory and daily-living activities, providing assistive care as required.
  • Collaborate with physicians to assess evaluations and test results, tailoring treatment plans to meet patient needs.
  • Maintain comprehensive documentation of patient care services, adhering to federal, state, and local regulations.
  • Conduct follow-up visits for home health orthopedic patients, ensuring continued eligibility for services.
  • Provide necessary maintenance therapy for ongoing patient improvement.
  • Travel to patient locations as part of the job requirement.

Qualifications:

  • Completion of an accredited Physical Therapist Assistant Program.
  • Active in-state licensure as a Physical Therapist Assistant.
  • At least one year of experience in home health care as a Physical Therapist Assistant.
  • Excellent interpersonal skills with a compassionate and empathetic approach.
  • Valid driver's license and a clean driving record.
