JOB SUMMARY

This position works in collaboration with the Director of the Hospital to ensure the integration of social services/case management functions into the patient care, discharge, and home-planning processes with other hospital departments, external service organizations agencies, and healthcare facilities as needed.

 

Responsible for developing and maintaining quality improvement programs, collaborating with physicians and medical staff to ensure service expectations are being met, and directing the workflow of the social workers.

 

PRIMARY JOB RESPONSIBILITIES

 

1. Assist the organization in overseeing the day-to-day operations of the hospital's social services department.

2. Assist the organization in the planning, preparation, implementation, and evaluation of a comprehensive social work program and processes related to facility Trauma & Stroke Programs.

3. Functions as the first-line professional lead, setting and monitoring compliance with professional standards under their supervision.

4. Meets frequently with Behavioral Health leadership to coordinate social work services/initiatives within the Behavioral Health Unit.

5. Provide clinical guidance and administrative supervision to social workers within the organization.

6. Coordinates and assists leadership with discharge planning initiatives using the health care team philosophy practice model.

7. Collaborates with other hospital disciplines and agencies/facilities in the community in the development, implementation and accomplishment of the goals for optimum hospital and post-hospital care; monitors the discharge plan and evaluates progress toward plan goals, making any necessary changes, as indicated.

 

ADDITIONAL JOB RESPONSIBILITIES 

 

1. Facilitate BHU therapy/case manager meetings.

2. Ensure compliance with CMS, State, and Mental Health Code standards and regulations by means of daily real time chart audits. Chart audits will monitor: Plan of care, Progress notes, D/C plans, and Psycho-social assessment completion within 72 hours.
