PHARMACIST - PER DIEM/DAYS

Location: King City, CA, 93930

Base Pay: $68.44 - $90.51 / Hour

Employee Type: Per Diem

Minimum Experience: 1 Year

Required Degree: 4-Year Degree

 

JOB SUMMARY: The Staff Pharmacist is responsible for providing the pharmacy department's clinical judgment, professional; knowledge, and distribution skills to assure the patients receive pharmaceutical care consistent with the mission of the department and the hospital. The Staff Pharmacist identifies, resolves, and prevents drug-related problems. These activities are performed within the context of established policies, procedures, and techniques of the Hospital, and the federal and state standards and regulations. The Staff Pharmacist supports and contributes to the service excellence mission of the Hospital and the Pharmacy department.

 

PERFORMANCE DIMENSIONS AND TASKS

Essential Function

1.   Dispensing: Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.

a.   Interprets medication orders (Verbal and written) and transcribes to computerized patient medication profiles accurately.

b.   Issues controlled substances to patient care areas and maintains records as required by law.

   

2.   Monitoring: Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.

a.   Monitoring drug therapy regimens for contraindications, drug-drug interactions, and allergies.

b.   Assesses patient for appropriateness of dose, route, and form of medication.

c.   Assists with pharmacokinetic consult service and drug dosing on request.

d.   Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.

e.   Reports suspected adverse drug reactions to the Risk Manager.

   

3. Position-specific competencies

a.   Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy.

b.   Provides accurate, adequate, and timely drug information to the professional staff.

c.   Documents all clinical activities and interventions accurately and completely via the department MUE program.

d.   Assess, identify, and address, on an ongoing basis, the special needs of all ages of patients when providing patient care.

e.   Screens drug appropriateness for age and physical systems tolerances.

f.   When required, collaborates with nursing to evaluate the effectiveness of therapy considering the age of the patient.

   

4.   Quality and efficiency: contributes to the quality and effective operation of the pharmacy.

a.   Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.

 

5.   Drive to an off-site clinic(s) to perform monitoring and medication stocking duties.

 

6.   Perform other duties as assigned.

Requirements

 

QUALIFICATIONS

Education:   Graduate of an Accredited School of Pharmacy preferred.

Work Experience:   Minimum of 1 year of hospital experience in an inpatient, decentralized setting preferred.

Computer experience with pharmacy programs and with Windows.

 

Skills Requirements:   Demonstrated application of drug knowledge and clinical judgment.

Licensing Requirements:   Current license as a Registered Pharmacist in good standing by the State of California.

 

Physical Demands:   Must be able to sit for 4 hours and stand/walk for 6 hours or more per day. Must be able to reach overhead and lift/carry up to 50 pounds. Moderate exposure to hazardous waste and materials.

Special Considerations:   Will be dealing with patients aged 0 to 120 years

 

Special Demands:   Ability to work independently with minimal direct supervision. Ability to work cooperatively with health system and pharmacy staff and communicate effectively both orally and in writing. Ability to handle frequent interruptions and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond quickly to emergency requests. Ability to exercise sound professional judgment. Must have the ability to perform the essential functions of the position.

 

Special Requirements:       Must sign Hospital Drivers Agreement

Provide a Valid California Driver’s License – upon hire and prior to expiration.

Provide Current DMV Records – upon hire and annually during the anniversary month.

Provide Proof of California mandatory limits of liability motor vehicle insurance – upon hire and prior to the expiration
