This role involves conducting in-person assessments and collaborating with an interdisciplinary team to develop and oversee individualized care plans. Responsibilities include managing patient care in nursing homes and hospitals, coordinating admissions and discharges, and consulting with medical specialists to ensure seamless care. Additionally, the physician provides guidance to nurse practitioners and other care team members.

Key Responsibilities:

  • Perform assessments, care planning, and provide ongoing medical oversight.
  • Collaborate with community physicians and IDT on discharge planning.
  • Participate in IDT meetings to coordinate comprehensive, 24-hour care.
  • Conduct regular visits to nursing home patients based on medical needs.
  • Participate in rotating night and weekend call coverage.

Qualifications:

  • Doctorate degree with successful completion of an accredited residency.
  • Board certification in Internal Medicine, Family Practice, or Geriatrics (preferred).
  • Active, unrestricted California Physician License, CDS certification, and DEA registration.
  • Eligibility for Medicare and Medi-Cal enrollment.

Compensation & Benefits:
